Check to see if:
1) the CTI_Gateway (the one being used in the CTI Profile) in the CUPS Configuration (Under the Application Menu) has the IP Address of the Publisher/Subscriber and not the name of the server
2) change the name to ip address in CUCM under Enterprise Parameters.... an implementation where the DNS is not present or not properly configured could result in the CTI Gateway not being resolved to an IP Address and thus not being reached.....
3) make sure the Roles for the CtiGw Application user in CUCM has the CTI Enabled/CTI Allow Control of All Devices and the End Users of the phones as well have Allow CTI Control....
